About the Role
Position Overview
The Community Coordinator (Scarborough Region) is a dynamic, localized role focused on building strategic relationships and expanding our footprint across Scarborough. You will serve as the primary face of our home care agency, establishing deep connections with local hospitals (such as Scarborough Health Network campuses), rehabilitation centers, family health teams, and culturally diverse community organizations.
Your goal is to build strong referral pipelines and seamlessly guide families through the transition from hospital or facility care to premium, safe home care. This position offers flexible scheduling options, accommodating both Part-Time and Full-Time tracks depending on the candidate's availability and regional demand.
Key Responsibilities
1. Scarborough Territory Management & Outreach
• Build and maintain strong referral pipelines with discharge planners, social workers, and transitional care teams across the Scarborough Health Network (General, Centenary, and Birchmount hospitals).
• Engage with local Ontario Health Teams (OHTs), community health centers, and family health networks across Scarborough to position our agency as a preferred transitional care partner.
• Cultivate relationships with diverse cultural and language-specific senior organizations, community hubs, and places of worship to ensure our care services are accessible to Scarborough's multicultural population.
• Represent the agency at local health fairs, senior expos, and networking events across the region.
2. Client Intake & Transition Support
• Act as the local point of contact for new inquiries and hospital referrals within the Scarborough region, providing empathetic, prompt guidance to families.
• Coordinate smooth hospital-to-home transitions, working closely with hospital discharge teams and our internal clinical staff to ensure care plans are safely implemented the moment the client arrives home.
• Conduct initial community-based consultations or family meetings to explain our specialized care models, including neuro-degenerative care and premium support services.
3. Documentation & Growth Tracking
• Manage and track outreach activities using our CRM, maintaining meticulous records of referral sources, accounts visited, and community engagement metrics.
• Identify gaps in local senior care and feedback trends to help our agency tailor specialized programs specifically for Scarborough residents.
Details
Required:
Experience: 2–4 years of experience in healthcare outreach, community coordination, medical sales, or social work. Direct experience navigating the Scarborough healthcare ecosystem or GTA home care market is a massive asset.
Local Knowledge: Deep familiarity with Scarborough’s geography, neighborhoods, and community resources.
Communication: Exceptional interpersonal, public speaking, and active listening skills. Ability to communicate effectively with both busy hospital clinicians and worried families.
Mobility: A valid Ontario G driver’s license and a reliable, insured vehicle for daily travel across the Scarborough region.
Tech Savvy: Proficiency with CRM tracking tools, scheduling platforms, and standard digital office suites.
Preferred:
Language Skills: Fluency in a second language widely spoken in the Scarborough community (e.g., Cantonese, Mandarin, Tamil, Tagalog, or Urdu) is considered a tremendous asset.
Education: An educational background in social work, gerontology, healthcare administration, or nursing/allied health.
