About the Role

Job Overview:
We are seeking a compassionate and dedicated Community Support Worker to join our team. The successful candidate will be responsible for providing assistance and support to individuals in the community, enabling them to live fulfilling and independent lives. This role requires a commitment to fostering positive relationships and helping individuals achieve their personal goals.

Key Responsibilities:
• Provide direct support and assistance to clients in various settings, including homes, community centers, and day programs.
• Assist clients with daily living activities, including personal care, meal preparation, and household management.
• Develop and implement individualized care plans in collaboration with clients, families, and other professionals.
• Foster social connections and community engagement by facilitating group activities and individual outings.
• Monitor and document client progress, as well as any changes in behavior or health status.
• Advocate for clients’ needs and rights within the community and ensure access to available resources.
• Maintain accurate records and documentation of services provided.
• Participate in team meetings, training sessions, and professional development opportunities.
• Uphold confidentiality and adhere to all policies and procedures concerning health and safety.

Details

Education:

  • Minimum of a High School Diploma or College Diploma or GED.

  • Certification in Community Support Work, Social Work, or a related field preferred.

  • Relevant college degree (Associate's or bachelor’s in social work, Psychology, or Human Services) is a plus.


Experience:

  • Previous experience in a support role, particularly in community service, social work, mental health, or related fields is advantageous.

  • Experience working with diverse populations, including individuals with disabilities, mental health challenges, or the elderly.

  • Skills and Competencies:

  • Strong interpersonal and communication skills, with the ability to build rapport with clients and their families.

  • Empathy, patience, and a commitment to promoting the dignity and independence of clients.

  • Problem-solving abilities and the capacity to handle challenging situations effectively.

  • Ability to work independently and as part of a collaborative team.

  • Proficiency with basic computer applications and documentation practices.

  • Certifications:

  • CPR and First Aid certification (or willingness to obtain).

  • Additional certification in Crisis Intervention or Mental Health First Aid is a plus.

  • Background check and/or screening may be required.
