ABOUT THE ROLE
As a Community Registered Nurse, you'll provide clinical assessments, care planning, and nursing oversight for seniors and adults receiving care in their homes throughout Newmarket, Aurora, Bradford, and surrounding communities.

Beyond providing direct nursing care, you'll play an important role in educating and supporting our Personal Support Workers. You'll help ensure every client receives safe, consistent care by coaching team members with varying levels of experience, answering clinical questions, and promoting best practices in home care.

Working closely with clients, families, physicians, therapists, pharmacists, and caregivers, you'll help clients remain safe, healthy, and independent while giving families confidence that their loved ones are receiving exceptional care.

RESPONSIBILITIES
• Complete comprehensive nursing assessments
• Develop and update individualized care plans
• Conduct follow-up assessments when client needs change
• Review medications and provide client education
• Assess mobility, falls risk, cognition, nutrition, skin integrity, and home safety
• Provide wound care and other specialty nursing services as required
• Support clients receiving oxygen therapy, catheter care, ostomy care, tube feeding, etc.
• Educate clients and families on disease management, safety, and healthy aging
• Mentor, educate, and support Personal Support Workers with varying levels of clinical experience
• Promote clinical best practices and assist with caregiver education when required
• Collaborate with physicians, therapists, pharmacists, and other healthcare professionals
• Maintain accurate electronic documentation
• Monitor changes in client condition and recommend appropriate interventions
