About the Role
The Pharmacist reports to the Pharmacy Manager. As an integral part of our interdisciplinary primary care team, dedicated to patient-centered care, your focus will be to provide medication distribution and drug information services.

REACH offers a widely respected interdisciplinary approach to care within our medical, dental, UPCC, pharmacy and multi-cultural family centre.
Responsibilities
• Consults with physicians, nurses and other health care providers to develop and discuss therapeutic plans appropriate for each patient based on chart review and assessment of clinical responses. Ensures appropriate documentation is maintained in the patient health record in accordance with regulations, protocols and practice standards.
• Provides patient medication counselling regarding drug therapy by reviewing reference literature, summarizing information and meeting with patients and their families to provide and explain information and plans, demonstrating procedures, techniques and use of equipment, and assessing patient's/family's understanding of the procedures, techniques and equipment.
• Provide pharmaceutical care and dispense medications to various clients
• Collaborate with other health care professionals to review and implement care plans
• Work with clients who are often medically complex, presenting with chronic disease, concurrent disorders or communicable diseases (e.g. diabetes, mental health issues, substance misuse, etc.) in an empathetic and patient manner.
Qualifications and Skills
• Current registration as a Pharmacist with the College of Pharmacist of British Columbia
• Must have injection certification.
• Must have OAT certification.
• WinRx experience is an asset.
• Clinical experience is an asset.
• Excellent critical thinking, time management and written and oral communication skills
• Strong relationship building skills as you become part of this high functioning team
• Self-directed, expert in your field.

• Ability to speak more than one language an asset.
